Chronic Disease Management

The Drayton Valley PCN’s Chronic Disease Management Program is currently under development. However, once this program is operating the PCN, working closely with family physicians, will provide services to support those patients dealing with chronic diseases. As with all PCN programs, these services are offered free of charge.

Initially the PCN will focus on the following chronic diseases:

  • Diabetes

  • Obesity

  • Smoking

  • Hypertension (high blood pressure)

  • Dyslipidemia (high cholesterol)

The PCN will offer:

  • Resources for newly diagnosed patients such as information about the disease, and newly developed research

  • Counselling to help deal with the effects of living with a disease

  • Education on tools and strategies to manage the disease effectively such as nutrition, medication management, and exercise

  • Regular follow-up to ensure the most appropriate medical action is being taken

The Drayton Valley PCN works closely with our group of family physicians and other health programs in the area to ensure that seamless care is offered to patients with chronic diseases
