Term March 15/23-March 31/24 with possibility of extension

 Position:  As a member of the Inter-Professional Primary Care Team the Chronic & Complex Care Nurse works at full scope of practice.  This position is responsible for the coordination of screening and prevention programs, provides support to physicians in the management of patients with chronic and complex conditions and works with others to support the ongoing development and evaluation of the Drayton Valley Primary Care Network.

 Our ideal candidate has:

• A degree from an approved health care program.
• Current/active practice permit and registration with CARNA.
• Current Basic Life Support (BLS)/CPR certificate for Health Care provider.
• Familiarity of OHS policies and procedures and an understanding of their workplace safety responsibilities.
• An understanding of the Alberta privacy laws.
• 3-5 years’ experience in a nursing capacity (primary care experience, an asset).
• Experience in health education and chronic disease management.
• Experience using an electronic medical record and NetCare.
• Strong diagnostic and assessment skills.
• Experience and strong awareness of community-based care and resources.
• The ability to establish and nurture good rapport, and collaborative relationships with patients, families, service providers and professionals in the surrounding communities.
• A desire to either live in the rural community being served or at least a strong vested interest in the health of the community to become familiar with the needs, resources, and particular issues unique to each community and each patient.
