 Position:  As a member of the Inter-Professional Primary Care Team the Chronic & Complex Care Nurse works at full scope of practice.  This position is responsible for the coordination of screening and prevention programs, provides support to physicians in the management of patients with chronic and complex conditions and works with others to support the ongoing development and evaluation of the Drayton Valley Primary Care Network.
