Chronic Disease Management

The Drayton Valley PCN’s Chronic Disease Management Program works closely with family physicians, providing services to support those patients dealing with chronic diseases. As with all PCN programs, these services are offered free of charge.

 

 

The PCN focuses on the following chronic diseases:

  • Diabetes
  • Obesity
  • Adult Asthma
  • COPD (Chronic Obstructive Pulmonary Disease)
  • Heart Failure
  • Hypertension (high blood pressure)
  • Dyslipidemia (high cholesterol)

The PCN offers:

  • Resources for newly diagnosed patients such as information about the disease, and newly developed research
  • Counselling to help deal with the effects of living with a disease
  • Education on tools and strategies to manage the disease effectively such as nutrition, medication management, and exercise
  • Regular follow-up to ensure the most appropriate medical action is being taken

The Drayton Valley PCN works closely with our group of family physicians and other health programs in the area to ensure that seamless care is offered to patients with chronic diseases
